Queen Square Family Health Team (QSFHT) provides primary health care, including chronic disease management, health promotion and disease prevention, to a rostered population of patients in the city of Brampton and surrounding area. The FHT employs an inter-professional team of health care providers who work in a collaborative model of care.

Position Summary:

QSFHT is currently seeking a Clinical Program Manager who will play an important role at the middle management level between the Interdisciplinary Health Providers (IHPs) and Executive Director. The Clinical Program Manager is responsible for the development, planning and evaluation of clinical programs and services provided by the FHT. The successful incumbent will be responsible for the implementation of operational processes, ongoing measurement and reporting to ensure the FHT operates efficiently and is compliant with all relevant legislative and statutory requirements. The role also includes management of the Administrative Assistants (4) to ensure an effective process flow that is conducive to patient- centered and efficient services.

Responsibilities

Administration:

  • Works collaboratively with the clinical teams to provide practice management support, coordination of services and support practice efficiency
  • Responsible for the development and update of clinical and operational policies and procedures
  • Collects, compiles and records measurable data on programs for quarterly evaluation
  • Leads the development of Quality Improvement Initiatives
  • Develops and monitors patient’s surveys
  • Supports the operationalization of the strategic goals
  • Leads and/or participates in various working groups
  • Ensures full compliance in the Ontario Occupational Health and Safety Act and all other applicable legislations
  • Understands and oversees Quality Improvement and Operational Efficiency
  • Supports the goals of providing person- centered care, improved access and the provision of quality care

Information Technology:

  • As the subject matter expert for the Electronic Health Records system (Accuro), provides leadership in the optimization of its use, administration, improvement and maintenance including:
  • Provides support to FHT users of the EMR system at all sites; troubleshooting, co-ordination of training;  design; Enhances process flow; Establishes new measurement systems if/where possible; Supports FHT staff to record, collect and analyze data; and Monitors privacy issues respecting PHI data and information sharing

Community Relations:

  • Identifies and cultivates linkages and partnerships with relevant community members and health care institutions,
  • Participates in building a creative supportive environment within the family health team
  • Represents the mission, values and philosophy of the FHT in the community, including networks, meetings or presentations, as requested.

Qualifications and Requirements:

  • Degree in a health related discipline; business or health administration with experience in healthcare
  • Demonstrated working experience in project leadership, coordination, program development, and procedure implementation
  • Certification in project management would be an asset
  • Excellent analytical, organizational and communication skills
  • Ability to work co‐operatively as a member of the FHT
  • Proficiency in the use of computers and various software applications
  • Proven flexibility and ability to adapt positively to change
  • Knowledge of electronic medical record (EMR) systems an asset
  • Strong analytical skills
  • Strong information technology skills an asset (hardware & software; design, development, troubleshooting, network configuration) systems and applications
  • Knowledge of relevant privacy legislation

Competencies, Skills and Abilities:

  • Knowledge of primary health care models in Ontario including Family Health Teams
  • Excellent communication skills (verbal and written) with the ability to formulate and articulate ideas and policy options clearly
  • Takes a proactive approach and organized to ensure priorities are met
  • Demonstrated ability to be innovative and flexible
  • Current CPR and First Aid certification required or to be obtained
  • Joint Health and Safety Committee certification an asset
  • Successfully passes a Vulnerable Sector Search
