The Association of Family Health Teams of Ontario (AFHTO) is a not-for-profit association representing Family Health Teams (FHTs) in Ontario. AFHTO works to support the implementation and growth of FHTs by promoting best practices, sharing lessons learned, and advocating on behalf of all FHTs.
While there has been a number of different models of primary care implemented over the years, it is our belief that the FHT model has the greatest potential to deliver truly patient-centric care in an Interprofessional setting. Being seen by the right health care provider at the right time ensures that primary care practitioners can see more patients, thereby reducing the number of unattached and orphan patients in the province and ensuring excellent care for all Ontarians.
