What are FHTs

Selecting a model that best fits your community

FHTs provide primary health care services in a team approach which brings together your family physician with other health care providers in order to provide to you the best possible care. The focus is on keeping you and your family healthy, not just treating you when you are sick. 

FHTs represent three different governance models:

  1. Provider-sponsored model: not-for-profit corporation governed by a Board of Directors primarily led by the sponsoring physician organization;
  2. Community-sponsored model: community not-for-profit corporations governed by a Board of Directors including local community representation; and
  3. Mixed-governance model:  mix of community and provider based groups coming together to form a not-for-profit corporation with a Board of Directors.
